The changes are drawn on top of the existing page rather than reflowing it, which is why they can be placed anywhere and why the rest of the document is left exactly as it was. Text is added as new text, so it stays selectable and sharp rather than arriving as a picture of words. White-out is a filled rectangle, drawing and highlights are vector shapes, and an image is embedded at the resolution you supply it at. Everything is written into a new copy when you save.
Where it stops: This adds to a page; it does not reflow one. You cannot retype a sentence in the middle of an existing paragraph and have the rest move to make room, because a PDF stores positioned text rather than a document that flows. For that, convert to Word first, edit, and convert back. White-out covers content visually but leaves it in the file, so use Redact PDF when something genuinely has to be gone.
