What happens to your file.
The ICO directory is read in your browser and every entry is checked against the image it points to. PNG entries are shown as they are; bitmap entries are decoded from their colour data and transparency mask, including the palette formats older Windows tools write. New sizes are drawn from the source you choose, and the saved icon is read back before download so it always opens.
Where it stops: Adding a size larger than the biggest image in the icon enlarges it, which looks soft; add large sizes from a bigger picture instead. An .ico can only hold square images up to 256 px, so non-square or larger entries can be saved as PNG but are left out of the new icon. Compressed bitmap entries, which are very rare, cannot be decoded.
